I/we certify that the particulars I/we will supply are true and correct, that I/we am/are over the age of 18 years and I/we am/are not an undischarged bankrupt. This information is given to Canterbury Finance Limited in the knowledge that reliance is placed upon it in accepting this application for credit.

purpose of the information I/we provide is for you to assess our
applications for finance and if applicable insurance cover from
Southbury Insurance Ltd and/or Lumley Life (NZ) Ltd and/or Lumley
General Insurance (NZ) Ltd and/or any other insurer to whom I/we apply
for credit-related insurance ("insurers");
- You and/or the insurers
may now or in future make inquiries about me/us to credit reporters
such as Baycorp and may pass my/our information to them, and they may
pass information about me/us to you;
- You may also disclose the
operation of our account or of any defaults by us to credit reporters,
and you, they and the insurers may hold and use my/our information in
the course of their businesses, including disclosure to third parties
in credit reports;
- I/we also authorise you to disclose the
operation of our account at any time to any member of the South
Canterbury Finance Group or to any dealer or broker involved in our
application, and authorise them and others to provide information
about us to you;
- I/we understand that under Privacy Act we have the right to access our information held by you, credit reporters, or the insurers, and the right to correct it.
